1. The complaint
Dear Sir / Madam,
I write to request a review under Schedule 3 to the Police Reform Act 2002 of the handling and / or outcome of my complaint, concluded by [FORCE] on [DATE].
My original complaint concerned [SUMMARY — e.g. failure to investigate reports of domestic abuse, failure to arrest despite a positive DASH risk assessment, treatment by attending officers, failure to safeguard a child].
2. Why the outcome is not reasonable and proportionate
I am dissatisfied for the following reasons:
1. [REASON — e.g. material evidence (body-worn footage, 999 call recording, witness account) was not obtained or considered].
2. [REASON — e.g. the College of Policing APP on Domestic Abuse and / or the force's own DA policy was not applied].

3. What I am asking for
I ask the IOPC to:
• find that the outcome was not reasonable and proportionate;
• direct the force to reinvestigate the matters not addressed;
• consider whether any officer's conduct meets the threshold for misconduct proceedings;
• make organisational learning recommendations to [FORCE] in relation to its handling of domestic abuse.
