1. Background
Dear Sir / Madam,
I have reported domestic abuse to your force on the following occasions: [DATES AND REFS]. A DASH risk assessment was completed on [DATE] by [OFFICER / IDVA] which scored [NUMBER] / 24, and / or recorded professional judgement of high risk.
Despite that, I have not been notified of a referral to MARAC. I am writing to ask:
1. Was a MARAC referral made? If not, why not?
2. If a referral was made, on what date, and what is the next scheduled MARAC?
3. Has an IDVA been allocated? If so, please share their contact details.
4. What safety planning, target hardening and Clare's Law disclosure has been considered?
2. What I am asking for
• That a MARAC referral is made (or progressed) without further delay.
• Allocation of an IDVA.
• Confirmation in writing of the action being taken under your force's Domestic Abuse policy and the College of Policing APP on Domestic Abuse.
• A copy of the DASH risk assessment held on the case file (subject access request — see separate template if required).
