Please provide me with a copy of all personal data you hold about me, including (but not limited to):
• all incident / crime / CAD / CRIS / STORM records in which I am named (whether as victim, witness, suspect or otherwise);
• body-worn video and any 999 / 101 audio in which I appear or speak;
• PNC, intelligence and Athena / Niche records;
• DASH risk assessments and MARAC referrals / minutes relating to me;
• all correspondence (paper or electronic) referring to me.
